Guest or Borrower understands the below terms and conditions:

1. The rental fee charged is non-refundable.
2.  An Items on Loan Rental Agreement should be signed along with this request form.
3.  All consoles/players are in the complete set and fully functional condition. 
4.  All CDs are scratch-free and functional.
5.  Any malfunctions and or insufficient conditions must be reported immediately in writing on the back side of this form and signed by both parties. 
6.  Available to borrow for 2 days.
7.  If there are no other requests for the console/player, the borrowed period can be extended for 2 days at a time.
8.  A deposit of $_____USD will be taken by the Front Desk.
9.  If the iPad, iPad cover, dock connector to USB cable, 10W USB power adapter, or any part thereof is missing or damaged upon return, the renter will be responsible for the AppleCare deductible or current replacement cost (whichever applies).
10.  Guests may separately purchase and install personal applications on the device, or console provided they are aware that they alone are responsible for all activities performed while using the application.
11.  Please do not attempt to add, delete, access, or modify other user accounts on the iPad, Xbox, game console, etc.
12.  Please do not remove the installed content on the iPad, Xbox, Playstation, etc.
13.  Any charges due to Apple or third parties while installing or updating the application should be paid by the guest directly. 
14.  Please be advised loss or damage to the console/player is charged at $_____USD.
15.  A maximum of 2 Games/DVDs is available for selection during console/player sign-up.
16.  Please note loss to the Game/DVD is charged at $_____USD per disc.
17.  Please also note loss or damage to the console/player equipment is charged at $_____USD per unit.
18.  Please do not remove or modify any identification labels that have been placed on the iPad, gaming console, Gaming CD, Playstation, Xbox, etc.
19.  Please do not add any stickers, labels, tags, or markings to the rented device.
20.  Please use the rented device appropriately which includes never removing any protective case, screen protectors, or other protective casings or covers.
